METHODS: Pachometry measurements were taken on consecutive Caucasian patients with IOP's of 21mm or above in four primary care optometric practices. These patients had no previous ocular surgery, recent contact lens wear, medications that would impact on corneal thickness or IOP, corneal endothelial disease or cup-to-disc ratio over .5. The patients were age and sex matched with patients who had IOP, s of 16mm or below. Goldmann tomometers and Sonogage pachometers were used to take measurements. RESULTS: The results show a significant difference in pachometer readings between the OHT and the hypotensive patients. The average corneal thickness for the OHT patients is 597mm while for the hypotensive patients it is 553mm. IOP for the patients with thick corneas show an increase of an average of 2.8mm of pressure over time while the hypotensive patients show none. CONCLUSIONS: This study confirms that pachometry readings taken in the primary care optometric practice show the same relationship between corneal thickness and IOP as has been found in research settings. This finding suggests that knowledge of corneal thickness is necessary to monitor and evaluate patients for glaucoma risk and diagnosis. Of more significance is the finding that IOP climbs in the thick cornea patients over time. This suggests an age related change in corneal rigidity which impacts on IOP measurements.
